Getting Started

In order to be considered for services, an application must be completed with a sponsoring Lion and supporting Lions Club. A full financial form must also be completed. Proof of information should be shown to the sponsoring Lion. Applicants do not need to mail or email that proof to MTLSS.

Please make sure that the application and financial forms are filled out completely. Incomplete forms will not be considered for services. Applications with falsified information will be denied.

The sponsoring Lion will be notified when the application is received. The application will then go before the Application Review Committee. The Application Review Committee meets monthly.

The sponsoring Lion will then be notified of the results of the Application Review Committee. We make decisions based on MTLSS policies.

The most important of these is working within the State of Tennessee Poverty Guidelines. Applicants must realize that our funds are limited.

We try to make decisions that will benefit as many people as possible. Although we would like to help everyone, with the funds we have we cannot cover everyone. We try to make decisions that will benefit as many people as possible.

If the application is approved, it will be sent in to make an appointment. Once the appointment date is set MTLSS will notify both the sponsoring Lion and the Applicant of their appointment date, time, and place.

The applicant will be sent the paperwork needed by the doctor’s office. We will also send the patient directions to the facility. They need to arrive 30 minutes before their appointment time.
The first appointment is an evaluation. Surgery will be scheduled at a later date if it is needed.

The approval is for the initial appointment and evaluation and cataract surgery of one eye at the doctor and facility appointed. If surgery is needed in both eyes, we may ask the club or the patient to make a donation or partial payment.

If the applicant needs further services or needs to be sent to another doctor, we will have to get estimates of the cost and re-evaluate. We cannot guarantee that we can cover all additional services.

Anyone making more is considered Over Scale by the poverty guidelines.  However, Middle Tennessee Lions Sight Service examines each case on an individual basis.
 
All applicants need to provide proof of current income to the sponsoring Lions Club. Proof may be in the form of a current income tax return, copy of last payroll check, or social security check.
