Application Process for Applicant

1. Complete the Application:
Fill out the entire application form. This application is valid for six months. Any services needed after six months will require a new application and approval.

2. Financial Information:
Complete the financial portion of the application and include proof of income. Preferably the last IRS filing, however you can include check stubs from last two paychecks. If you draw SS or SSDI, include the form showing your payment. You do not have to include proof of your bills.

3. Accuracy and Honesty:
Ensure the form is completely filled out. Forms that are not fully completed will not be eligible for consideration of services. Providing false information will result in your application being denied.

4. Review Process:
The application will be reviewed by the Application Review Committee. You will receive a notification regarding the committee's decision. Decisions are based on MTLSS policies, primarily working with the State of Tennessee Poverty Guidelines. Please understand our funds are limited, and we strive to make decisions that will benefit the most people possible. Although we would like to help everyone, we cannot cover everyone with the funds we have.

5. Appointment Scheduling:
If approved, the application is sent for an appointment. Once the appointment date is set, MTLSS will notify you of the
appointment date, time, place, and the doctor you will be seeing. You will be notified promptly once your appointment has been scheduled.

6. Appointment Day:
Please take your ID with you to your appointment. Also bring with you a list of your medications and glasses (if you have some). On the day of your appointment, bring the sheet that is sent to you with your appointment time and place. That paper serves as your coverage at Middle Tennessee Lions Sight Service. You must have that paper to be seen. Arrive about 30 minutes early for your appointment to allow time to complete the paperwork. The first appointment is your evaluation. Surgery will be scheduled later if needed.

7. Coverage Details:
Your approval covers the initial appointment, evaluation, and cataract surgery at the doctor and facility appointed. We cover anesthesia, surgicenter, and any physician fees. Any additional charges are your responsibility, including eye drops or additional testing.

8. Further Services:
If you need further services or referral to another doctor, we will obtain cost estimates and re-evaluate. Although we will try, we cannot guarantee coverage for additional services.

9. Behavior Expectations:
If approved, please be kind to the medical staff. They are helping us restore your vision. Vision is important, and kindness is important. The physicians assist us with reduced charges, but you will receive the same expertise and skill as if you paid extra. If you are rude, cuss, or are inappropriate, we will terminate your services.

Getting Started

You must fill out the entire application with a sponsoring Lion and supporting Lions Club in order to be considered for services. To find a Lions Club near you, Find a Local Lions Club | Lions Clubs International.

Additionally, you must complete the financial portion fully.  Proof of the information on the financial form needs to be shown to the sponsoring Lion. You do not need to mail or email that proof to MTLSS.

Please make sure that the form is filled out completely. Incomplete forms will not be considered for services. Providing any false information will result in your application being denied. 

The Sponsoring Lions Club will send the application to the Middle Tennessee Lions Sight Service, where the application will go before the Application Review Committee.

The Application Review Committee meets monthly. You will be notified of the decision of the Application Review Committee.

We make decisions based on MTLSS policies. The most important of these is working with the State of Tennessee Poverty Guidelines.

Please understand that our funds are limited and we try to make decisions that will benefit as many people as possible. Although we would like to help everyone, with the funds we have we cannot cover everyone.

If you are approved we will send the application in for an appointment. We do not have control over the time of the appointment scheduling.

Once the appointment date is set, MTLSS will notify both the sponsoring Lion and the Applicant of their appointment date, time, and location of the doctor you will be seeing. You will also be sent directions to the clinic where you have your appointment.

We will contact you as soon as the appointment is made.

Arrive 30 minutes early for your appointment to allow time to complete the doctor’s paperwork. The first appointment is your evaluation. Surgery will be scheduled at a later date if it is needed.

Your approval is for the initial appointment, evaluation, and cataract surgery of one eye at the doctor and facility appointed. If you need surgery in both eyes we may ask for a donation or partial payment from you.

If you need further services or need to be sent to another doctor, we will have to get estimates of the cost and re-evaluate. Although we will try, we cannot guarantee that we can cover all additional services.
